In addition to the SOPHAS required documentation of official U.S./Canadian transcripts and C.V./resumé, please provide the following additional documentation with your application:

• Official transcripts from each U.S. and English-speaking Canadian college-level institution attended
• Statement of purpose and objectives
  • Your personal statement should be two to three double-spaced pages in length and address the following:
    • What are your current public health/health informatics career goals?
    • How will a dual MSHI/MPH degree complement those goals?
    • Why would you like to study at Northeastern in particular?
• Three letters of recommendation
  • Academic and professional references ONLY
• Standardized test scores (one of the following)
  • GRE: have ETS send scores to SOPHAS using code #7533
  • MCAT: have AAMC send scores directly to SOPHAS
  • Waiver requests are available on our program’s website under “Official Test Scores”
• English proficiency test scores for those applicants whose native language is not English and/or who attended post-secondary institutions outside the U.S. and English-speaking Canada.
  • TOEFL: have ETS send scores to SOPHAS using code 5688
  • Waiver requests are available on our program’s website under “Official Test Scores”
• World Education Services (WES) ICAP course-by-course evaluation of academic records from institutions outside the U.S. and English-speaking Canada (if applicable)
